GENERAL REQUIREMENTS

Visa Application Form

• Personal appearance is required by appointment

• Visa application form

• Original valid passport at least 6 months valid from date of departure

• Photocopy of biopage / entry and exit point

• If employed - Employment certificate with salary compensation

• Photocopy of PRC ID (for professional). Company ID (for employee). IBP ID (for lawyer), and bring the original ID upon appearance

• If business owners / self-employed - DTI, SEC, Mayor's permit

• Xerox copy of latest income tax return

• Original bank certificate

• Original bank statement latest 6 months

• If attending a meeting, conference, or convention, registration of the event with daily schedule

• If invited - need invitation letter

• If sponsored, need a sponsorship letter from sponsoring company

• Photocopy PSA Birth Certificate

• Photocopy of PSA Marriage Contract (If Married)

FOR CHILD

• Submit documents of parents same as above together with affidavit of support

• If studying, submit enrollment certificate from school

• Photocopy of school ID

• Photocopy of PSA birth certificate

• IF travelling alone / travelling with guardian or another person, submit DSWD permit

NOTE:

SUBMISSION OF COMPLETE REQUIREMENTS 1 1/2 MONTHS BEFORE INTENDED DEPARTURE

Processing Time

Approximately 14 working days from the time application is received by the embassy (can be extended without prior notice).
